Do you or your child need a ride to your health care visits? Let us help you get there. You can get rides to services we cover.

Ambulance rides 


If you have no way to get to the hospital during an emergency, call 911. We cover ambulance rides (on the ground) in a medical emergency for all members. 

 

Nonemergency medical rides

 

We work with Modivcare to offer rides for nonemergency medical needs. They are rideshare partners with Lyft® and Uber Health®. You or your child can get a ride for visits to and from medical visits if you don’t have a way there. Covered rides include trips for services like:

When to ask for a ride

When to ask for a ride

Ask for a ride as early as you can, and at least: 

 

  • Two business days before you need it for regular trips 

 

Sometimes you may ask for a ride with less notice, like:  

 

  • Pick up after discharge from a hospital 
  • Trips to the pharmacy to pick up medication or approved medical supplies 
  • Trips for urgent conditions (not emergencies but severe or painful enough to need treatment within 24 hours)

Need to cancel your health care visit? You’ll want to let us know before the approved and scheduled trip.

How to set up a ride

Need help scheduling a ride? Call Modivcare at 1-866-824-1567 (TTY: 711). They’ll be there to answer your call 24 hours a day, 7 days a week.

 

What to expect when you set up a ride:

 

  • A care manager will gather details from you and then schedule your ride. 
  • If the ride is for a child age 18 or younger, a parent or guardian will need to fill out a rider consent form. (This only needs to be done for the first ride.)
  • You’ll get ride notices like reminders and alerts. You can choose whether you want them by text, phone or email.
  • You can list a care contact person, like a parent or a care manager. This person will also get the notices and alerts about your trip. 

 

Be sure to have this info ready when you call:

 

  • Your first and last name
  • Your date of birth
  • Your pickup address
  • Your phone number
  • Name of your provider
  • Date and time of your health care appointment 
  • If anyone is traveling with you (you can take one person as an attendant)
  • If you have special travel needs (example: wheelchair access)

Be aware of travel requirements for children

 

If your child is the one traveling, they must be:

 

  • Age 18 and older to travel alone
  • With an adult (18 and over) if they are under age 18
  • Please bring your own car seat for children that need one. Car seats must fit legal requirements in your city and state
